Totally confused about what to do next

Eight months ago, I started having pain and swelling in my hands (not neccesarily at the same time). Then 3 months ago, I started falling for no reason. After many doctor's visits and thousands of dollars later (without insurance), I have been told I have essential tremors and placed on a walker. The only test NOT run has been an MRI due to a pacemaker. They have me on medication that helps the tremors as long as I don't try to do anything more than 30 min long. The pain is persistant without relief from medication they have given me as of yet. I have lost my job and am unable to gain employment due to my condition. Any suggestions on where I might turn next would be greatly appreciated.

They did the spinal tap which came out normal. Any other suggestions?
